The following information is for new and returning NPU student-athletes, including the necessary medical forms that must be completed each year prior to participating in any intercollegiate practice, competition, or strength & conditioning session.

Please follow this guide closely to ensure you will be medically cleared to participate in athletics when you arrive on campus. This process is now completely online based utilizing our electronic medical record system.

Annual Physical
You will find that part of the pre-participation process is having an annual physical examination each year. This exam must be performed by a physician (M.D. or D.O). Physicals signed by a physician’s assistant (PA-C), nurse practitioner (NP), or other clinician will not be accepted, nor will exams that are performed at “walk-in” style clinics (e.g., CVS Minute Clinic). This is per NCAA rule.
When you attend your physical appointment, please make sure to bring a blank NPU Physical Form and your completed Medical History Form along with you. You can find instructions on how to locate and print these forms within the Medical Forms Guide posted above.
Health Insurance Requirement
Before participating in any intercollegiate sport activity at North Park, all student-athletes must submit a copy of their current insurance card(s). Without a primary insurance plan on file, you cannot participate. You can find instructions on how to upload a front and back copy of your insurance card within the Medical Forms Guide posted above.
All student-athletes at North Park are additionally covered by an Athlete Accident Excess Policy via First Agency, Inc. of Kalamazoo, Michigan.

Health Services Requirements
All new and transfer students, athlete or not, must complete the Immunization Record, which is required by, and should be turned in to, the Health Services Office. Find this information at the Health Services page. This is separate from the medical forms necessary for athletics and is only required your first year at North Park.
